Care Coordination
Life can change quickly after a major life event or aging itself can warrent connecting with resources. Knowing where to turn can feel overwhelming. Our team of Care Coordination helps older adults and their families navigate available resources and connect with services that support independence at home.
Our goal is simple: help older adults remain safely in their homes and communities for as long as possible.
Our knowledgeable Care Coordinators work one-on-one with older adults and their families to:
- Connect you with home- and community-based services
- Provide information about Medicare, health insurance, and available benefits
- Connect you with in-home services and personal care options
- Identify community resources that support health, safety, and independence
- Help you understand your options and make informed decisions
Care Coordination is available to adults age 60 and older who may be:
- Experiencing a health crisis or declining health
- Living alone or lacking adequate family support
- At risk of losing their independence
- Feeling isolated or facing economic or social challenges
- Looking for guidance in accessing services and community resources
Every person's situation is unique. That's why our Care Coordinators take the time to understand your goals, assess your needs, and connect you with the services that best fit your circumstances.
Through funding provided by the Older Americans Act and other funding sources, we help older adults and their caregivers access the support they need to continue living with dignity, confidence, and independence.
